We are always delighted to accept new patients who live in Westbury and the surrounding areas onto our practice list. If you wish to register with us, please visit either of our surgeries, speak to our reception staff and collect a registration pack. Alternatively, you can click on the three links below, print the forms and then bring them to your surgery.
You must provide photo ID and proof of address when you return your forms to us. If you are currently taking repeat medication please let us know when you return your registration forms. We will not accept forms posted or emailed to us. You must live within our contractual boundary in order for us to be able to provide ongoing care for you. Westbury Group Practice area covers Westbury Warminster, Dilton Marsh, Chapmanslade, Upton Scudamore, Heywood, Hawkeridge, North Bradley, Bratton, Edington, Erlestoke and Steeple Ashton.
Anyone staying at an address other than their normal residence for less than three months is entitled to NHS treatment if they develop a condition which is classed as an emergency or requires "immediately necessary" attention. However, everyone travelling away from home should ensure they have sufficient supplies of prescribed medicine to take during their stay. Doctors are entitled to charge a consultation fee and a private prescription charge if they are asked to prescribe routine medication.
If you are unwell while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient. You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
To register as a temporary patient with us, speak with our reception team and fill in a GMS3 form. Alternatively, you can print out the form below and bring it to your us. You will need to bring two forms of ID, a photo ID and a separate proof of address.

Named Accountable GPs
All patients registered with Westbury Group Practice have a named accountable GP who is responsible for their overall care at the practice. If you wish to know who your named GP is, please contact the practice. If you have a preference as to which GP this is, the practice will make reasonable efforts to accommodate this request.
