Many of you who are registered with the practice will be receiving a questionnaire over the coming months, asking you about your experiences of local health services.

This questionnaire is sent out on a quarterly basis by the Department of Health and covers issues such as access to services and satisfaction with treatment.

If you receive a questionnaire, please take the time to fill it in and send it back as soon as you can, even if you have received a questionnaire like this before.

The feedback will help the practice to improve the services provided to our patients.
