Many of you who are registered with the practice will be receiving a questionnaire over the coming months, asking you about your experiences of local health services.

This questionnaire is sent out on a quarterly basis by the Department of Health and covers issues such as access to services and satisfaction with treatment.

If you receive a questionnaire, please take the time to fill it in and send it back as soon as you can, even if you have received a questionnaire like this before.

Not only will the feedback help the practice to improve the services provided to our patients, it will also help to ensure that we are able to replace funding which has already been removed from us to support these questionnaires!
